Becoming a New Member

​Monthly rates are $300 a month for adults and $150 a month for children ages 5-17.
Please complete the enrollment form above to join the practice.  
​Our office will contact you to schedule an appointment once we receive your enrollment form.
Our practice does not accept or bill insurance.

You may choose to stop your membership at any time and be removed as an active patient.  
If you choose to rejoin, you will be placed on the waiting list for new patients.
